Complaint Procedures

Family Service Center Board of Directors has created a complaint procedure for the entire agency that can be found here.  We are a licensed child welfare agency in the State of Illinois, and our license number is 006840-13.   For complaints, please contact us at (217)528-8406 and ask to speak to the Executive Director.  Complaints and concerns can also be registered with the Illinois Adoption Register at (800)572-2390.
